The deadlines for submitting Med3 and Med4 forms are as follows:

Med3 Forms Due Date
1. Preference Sheets Usually one month before rotation
2. Mid-month Evaluations End of each month
3. Clerkship & Team Evaluations At or before final exam

Med4 Forms Due Date
1. Rotation change requests 10 days before rotation starts
2. Clerkship & Team Evaluations Before grade is sent to the College
